Are you between the ages of 21 and 40?
Have you had one or more medically uncomplicated pregnancies?
Are you healthy, and height/weight appropriate?
Are you a non-smoker, drug free, and alcohol free?
Have you never been convicted of a felony?
Do you have reliable transportation and live in Connecticut?
Are you a U.S. citizen or have a green card?
Are you not currently receiving public assistance?

If you can answer "no" to any of these questions, then we are sorry but you do not meet the qualifications. Please do not submit an application to become a surrogate.
